Describe
the obstetric management of abruptio placenta.
Obstetric management consists of vigorous
maternal resuscitation with fluids and blood replacement and delivery of the
fetus. In the presence of a small abruption and in the absence of fetal
distress a vaginal delivery may be attempted. In the presence of
life-threatening hemorrhage or fetal compromise, cesarean section is the most
expedi-tious route of delivery. Preventing and treating coagulopa-thy requires
evacuation of the uterus as well as transfusion of coagulation factors and
platelets, as indicated.
