What
types of irrigating fluids have been used for a TURP?
The ideal irrigating fluid would be isotonic,
electrically inert, nontoxic and transparent. Unfortunately, this type of
solution does not exist. Originally, distilled water was used. However,
absorption of distilled water caused hypona-tremia and hemolysis of red blood
cells. This severe complication led to the use of iso-osmotic solutions such as
saline or lactated Ringer solution. However, since these solutions are highly
ionized, they caused dispersion of the high-frequency current from the
resectoscope. The present generation of irrigating fluids is electrically inert
as well as isotonic. These solutions include glycine and Cytal (the most
commonly used) as well as glucose, mannitol, urea, and sorbitol.
