What
would you do in this case, if the patient could not be adequately ventilated
with a mask?
This clinical situation of local anesthetic
overdose and seizure resulting in respiratory insufficiency is likely
tem-porary and reversible. It is not related to the patient’s underlying
disease state directly. It would be imperative to secure the patient’s airway
and begin ventilation with either a mask or an endotracheal tube until baseline
mental status is restored. This would be consistent with honoring the patient’s
wishes rather than just procedure-directed therapy.
Many patients choose procedure-directed therapy
(list of procedures they will and will not accept). This works well to
alleviate anxieties but is often inconsistent or impossible to reconcile
medically. Goal-directed therapy is preferable in circumstances such as this.
