Why is it important to maintain sinus rhythm?
Atrial systole normally contributes about
15–20% to stroke volume. In AS, this increases to 40–50%. The atrial “kick” is
crucial in preserving left ventricular filling (and stroke volume) since
passive filling is decreased because of the noncompliant left ventricle. The
onset of a non-sinus rhythm is often associated with marked hypotension because
of the decrease in stroke volume. It is difficult for the patient with AS to
compensate for the loss of sinus rhythm because marked increases in left atrial
pressure would be required to maintain an adequate stroke volume.
